Liquor License Renewal Form

REQUIRED DOCUMENTS AND PAYMENTS TO BE SUBMITTED WITH THIS FORM:

1. Retail Liquor or Fermented Malt Beverage License Renewal Application DR-8400
2. Receipt of payment of state fees (Colorado Payment Link)
3. Tax Check Authorization, Waiver, and Request to Release Information DR-8495
4. Payment for the Town of Lakeside’s liquor renewal fees should be made by check, payable to the “Town of Lakeside”.

These forms and proof of payment must be remitted no later than 60 days before the expiration date.
